Diabetes requires self-management 24 hours a day, 7 days a week. Along with self-management, there are multiple decisions that need to be made including food choices, physical activity and medication dosing. Healthcare professionals have an opportunity to leverage technology to support people with diabetes to decrease the burden of living with a chronic condition and facilitate engagement in discovery learning. Continuous glucose monitoring (CGM) is an amazing tool to support people as they learn about their own diabetes and how their choices impact their glucose patterns.

Data generated by CGM may be unfamiliar to Healthcare professionals, but Dexcom CLARITY software simplifies decision making by recognizing and reporting patterns and trends. In this session we’ll discuss the benefits of using a personal Dexcom G6 CGM, how to identify individuals who may benefit from CGM data, and the workflow for a primary care clinic to be effective. We’ll also present a case study that shows how visualizing CGM data can support both health behavior changes and treatment modifications. We’ll show how CGM enables virtual visits via CLARITY clinic’s cloud-based data sharing platform. We’ll also discuss the new Dexcom G6 Professional, clinic owned system for intermittent CGM wear to uncover hidden glucose patterns. CGM technology is now the standard of care for people with type 1 diabetes and those with type 2 using insulin.
