Through Digital Health Partnerships, Dexcom empowers people with diabetes to engage in just‐in‐time care and healthcare professionals (HCPs) to support the 24/7/365 nature of diabetes with continuous care. Compared to random blood glucose checks, real‐time continuous glucose monitoring (CGM) data provides valuable opportunities to empower self‐management through discovery learning, facilitate shared decision‐making with the care team and generate actionable insights for coaching and treatment decisions. Diabetes Care and Education Specialists (DCES) can lead the integration and optimization of technology into the care plan and use CGM data to drive improved clinical and psychosocial outcomes.
