Through Digital Health Partnerships, Dexcom empowers people with diabetes to engage in just‐in‐time care and supports healthcare professionals (HCPs) to enable the 24/7/365 nature of diabetes with continuous care. Compared to random blood glucose checks, real‐time continuous glucose monitoring (CGM) data provides valuable opportunities to empower self‐management through discovery learning, facilitate shared decision‐making with the care team and generate actionable insights for coaching and treatment decisions. Diabetes Care and Education Specialists (DCES) can lead care team to integrate and optimize use of technology in the care plan and use CGM data to improve diabetes outcomes.
